Service Support Coordinator - FT (Day) - North Mankato, MN

Position details: Full-time schedule working 9:00-5:00 Monday-Friday.

As a Service Support Coordinator with Vista Prairie Communities, you will live the values of Caring, Respect, Collaboration, Innovation and Stewardship. This role will provide organizational, receptionist, and clerical support to the Assisted Living care team and function as a resource to members of the health care team. Create a meaningful opportunity for yourself by joining our strong Vista Prairie Communities team!

Key Accountabilities

  • Supports the health care team by anticipating and responding to team requests and patient needs.
  • Provides support to clinical staff.
  • Communicates effectively with patients, visitors, and community staff via a variety of modalities. Plays an integral role in communication throughout the community.
  • Transcribes orders to patients' electronic medical records.
  • Enters orders and files documentation.
  • Arranges transportation, schedules appointments and doctor visits, and prints off documentation for doctor visits.
  • Assists residents with Activities of Daily Living (ADL’s); i.e. dental and mouth care, dressing/ undressing, hair care, toileting, personal hygiene, and other duties as directed. Assists with residents on floor as needed.
  • Completes additional tasks as assigned.

Additional Skills and Qualifications

  • Basic knowledge of computer systems and strong keyboarding skills.
  • Good judgment and outstanding customer service, interpersonal, organizational, and problem-solving skills.
  • Ability to assume responsibility for self-development within the role.

Education and Experience

  • High school education or equivalent.
  • Successful completion of college, HUC vocational program, and/or HUC Certification preferred.

Requirements

  • Follows all policies and procedures.
  • Must be a minimum of eighteen (18) years of age.
  • Must be able to relate information concerning residents’ condition.
  • Must possess the ability to make independent decisions when circumstances warrant action.
  • Must receive a "not disqualified" criminal background result may not be on the OIG exclusions list.
  • Stays up to date on all required training.
  • Understands and adheres to the Vulnerable Adult and Abuse Policy. Is familiar with the Resident’s Bill of Rights and adheres to it.
